The full descript꧟ion for Dirge of the Fallen Forge reads, "Play the 'Lament of the Storm' at a friend's grave." If you've finished the main game, you've likely surmised that this f🍃riend is Taka. A noble friend, he dies in Act 2 while trying to save Jin from the clutches of Khotun Khan. This will be the second to last mission during Act 2.

After that, you'll wind up at Taka's grave with his sister Yuna. I've taken a picture of it above in case you forget where to locate it. Apart from the location, you'll need to have the "Lament of the Storm" song unlocked to get this trophy. You unlock son🦩gs by finding singing crickets, which are scatt𓆏ered around Tsushima.

It will take you only three crickets to get this particular song. You can speed this up using the traveler's attire and by unlocking the skill that lets you track crickets. After doing so, you'll need to equip the song in the game's menu and play it in front of Taka's g🏅rave. Yuna doesn't need to be there and you don't even need to have it before Act 2. If you want to take a small reprieve after his sacrifice, searching for crickets is even a good way to cool down.
