You're introduced to their return very early on. During one of the first main missions, you have to rescue J. Jonah Jameson from a limousine trapped in sand, thanks to Sandman's rampage through the city. Later on, when the world opens up, you can find a couple of friends lighting unlicensed fireworks on a rooftop. Miles Morales does his best British impersonation as he leaps down to ask if they've got a permit. They don't, and their plan literally blows up in one of their faces, so you have to rush him to the hospital.
There's no timer like back in the PS2 days so it's a lot more forgiving, and the mission goes beyond the simple delivery. Once he's at the hospital, you go off to scout the place where they bought the illegal fireworks and take down some crooks. There are a few more missions like this scattered throughout the world, bringing back and evolving the old Hospital Run encounters.
You can even find injured civilians during other encounters. For instance, I responded to a red crime icon and found some cultists kidnapping hapless bystanders. Once I dispatched them (read: slammed their heads into the ground at terminal velocity, not killing them - gotta stress that) I found one of the hostages was in need of medical treatment. I slumped him over m♍y shoulder and off we went to the hospital.

Luckily, the missions are less finicky than they used to be, since lugging someone around doesn't hinder swinging much at all. Both Spider-Men are strong, they can handle someone on their shoulders when they're zipping across the city.
