In Traverse Town, the starting area has the Inn and the Accessory Shop which Cid operates. At a certain point in the story, you'll get the ability to use Green Trinity marks. This shop is special in that it allows you to synthesize a variety of items from consumables to weapons. There are some items that can only be created in this shop and the Ultima Weapon is one of them.
Even though you'll likely gather the necessary ingredients throughout the adventure, you won't be able to create it right away. You will first need to unlock the required recipe in the Synthesis Shop.
The way the shop works is that it has lists of recipes that are 'unlocked'. You start off with one list, and you'll need to make your way through each recipe on it. Once you've crafted enough recipes on the list, you'll unlock a new list with new items to create.

You'll need to do this all the way until you reach the sixth list, which will unlock the Ultima Weapon recipe. To synthesize it, you'll need five Thunder Gems, five Mystery Goo blobs, three Serenity Power pieces, three Stormy Stones, and three Dark Matter pieces.
The most reliable way to get the required materials is from monster drops. Unfortunately, you may have to grind certain enemies in order to get them. From highest to lowest enemy drop probability for each material: Thunder Gem (White Mushroom, Bambi Summon, Aquatank, Screwdriver), Mystery Goo (Rare Truffle, Black ♑Fungus, White Mushroom)෴, Serenity Power (Pink Agaricus), and Stormy Stone (Neoshadow). Dark Matter is not dropped in Final Mix and must be synthesized with nine Lucid Shards (White Mushroom, Bambi Summon, Shadow), two Mythril (Bambi Summon, various Trinity and treasure chest locations), and one Gale (Angel Star, Invisible).
Once you have all this and the sixth list, you can finall🦂y synthesize the Ultima Weapon.
