Sheogorath has this oddly unique vibe to him when you first meet him in the Shivering Isles in The Elder Scrolls IV: Oblivion Remastered, and he will task you 🍰with doing a lot of things for him. But the more you progress, the more you realize that you've become caught up in performing certain rituals that 🦩the region undergoes every once in a while.

As part of this, you'll have to dispose of one of the two available Duke or Duchess in the region and take their place. However, you get to choose which one to replace and which side to align with. How much this matters and how it affects the story is something you'll discover as you dig deeper into the cycle you're ♎now part of.

How To Start The Ritual Of Dementia Quest

During the Ritual of Accession main quest of the Shivering Isles, you will have the option to choose which ritual you want to undertake when speaking to Sheogorath.

If you choose Syl, Duchess of Dementia, it will begin the Ritual of Dementia quest, where you will need to dispose of her and have her replaced.

How To Become The Duke/Duchess Of Dementia

Speak to Arctus so he can give you the first lead, which is to speak to her servant, Anya Herrick, and Kithlan to gather more information about her da🃏ily routine.

This will ꦫhelp you fꩵind the perfect opportunity to kill her.

Make sure to raise the servants' disposition to at least 70 before speaking to them, so they will spill the beans. Otherwise, you won’🀅t get any useful information.

Afterward, you will learn that Syl is hiding, but with the help ♕of both♛ servants, you’ll get closer to her.

• Anya will mention that she can distract two guards to help you get closer.
• Kithlan will give you the House Dementia Key, which allows you to navigate the doors without needing to lockpick them.

Make your way to the Private Gardens, defeat t🎃he two guards, and then head inside to the Duchess' Quarters.

At the bed, you will find Syl dead, but then the servants will approach and tell you it’s a trap to fool her enemies. She has escaped to a hidden passage.

This passage can be accessed by interacting with the statue in the northeast corner of the garden. This will take you to a secret cave, Xirethard.

The cave iඣs filled with Dark Seducer guards that will attack you on 🙈sight, along with traps, so be cautious. Follow the objective marker to reach the end of the cave.

At the end, you’ll find Syl prepared to fight you. Simply defeat her, then loot her body to obtain Syl's heart. With this item in hand, head back to Sheogorath at Sacellum Arden-Sul.

Activate the altar by placing the heart on it. A cutscene will trigger, and after it ends, speak to Sheogorath again to receive the Ring of Lordship as your reward, completing the quest.
