Being sick would cause characters to have lower stats, have an easier time being afflicted with status effects, and make them easier to knock down. It was very annoying, and only slightly offset by characters having a slim chance of being well-rested and having status improvements. Thankfully, it's not going to be an issue anymore, as Persona 3 Reload won't include the fatigue system at all.
